If CMI does not identify documented financial opportunity equal to at least our professional fee, we refund the difference.
- No percentage of savings.
- No back-end fee.
- No obligation to hire CMI for anything else.
02 — Fix it
Plan Correction & Implementation
Finding the problem is only useful if someone fixes it.
This engagement is for employers that need help correcting an existing health plan or replacing arrangements that no longer serve the company or its employees.
Fixed Fee
| Covered Employees | Fee |
|---|---|
| Up to 100 | $15,000 |
| 101–250 | $20,000 |
| 251–500 | $30,000 |
| 501–1,000 | $40,000 |
| 1,001+ | Fixed fee quoted in advance |
Work may include:

- Stop-loss strategy and placement support
- Vendor negotiations
- Contract correction
- Benefit design

- Implementation oversight
CMI remains independent throughout the process.
We do not receive compensation from the vendors we evaluate or recommend.
If an engagement requires work materially outside the agreed scope, CMI identifies and prices that work before proceeding.
No surprise invoices.
03 — Manage it
Fiduciary Management
For employers that do not want senior leadership attempting to manage a healthcare system they were never trained to understand.
Where CMI is formally appointed and provided the appropriate authority, CMI may accept defined fiduciary responsibility for specific health-plan functions.
Annual Fixed Fee — Billed Monthly
| Covered Employees | Annual Fee | Monthly Billing |
|---|---|---|
| Up to 100 | $50,000 | $4,167/month |
| 101–250 | $75,000 | $6,250/month |
| 251–500 | $125,000 | $10,417/month |
| 501–1,000 | $175,000 | $14,583/month |
| 1,001+ | Fixed annual fee | Billed monthly |
These are annual engagements billed in equal monthly installments.
CMI does not charge PEPM.
The fee does not increase because claims are higher, a large claimant appears, or enrollment changes modestly during the year.
Depending upon the engagement, CMI may:
- Independently oversee claims administration
- Review material and high-dollar claims
- Monitor pharmacy spending
- Oversee TPA and PBM performance
- Review vendor compensation
- Monitor stop-loss activity and reimbursements
- Review contracts and amendments
- Evaluate plan expenses for reasonableness
- Oversee provider reimbursement
- Maintain the employer's fiduciary file
- Document material plan decisions
- Provide monthly executive reporting
- Conduct formal quarterly fiduciary reviews
- Identify and address emerging financial or contractual risks
The employer receives access to its maintained fiduciary record and clear reporting based on:
Actual dollars.
Actual activity.
Actual decisions.
No meaningless dashboards.
No reports designed to make bad numbers look better.
Where did our money go?
CMI answers it.
Initial engagement
Initial Takeover & Implementation
If an employer comes directly to CMI for fiduciary management and CMI has not previously reviewed or corrected the plan, an initial takeover and implementation engagement may be required.
| Covered Employees | Initial Takeover Fee |
|---|---|
| Up to 100 | $25,000 |
| 101–250 | $30,000 |
| 251–500 | $40,000 |
| 501–1,000 | $50,000 |
| 1,001+ | Fixed fee quoted in advance |
No Duplicate Fees
If CMI has already completed the review, correction or implementation work necessary to transition the plan into fiduciary management, the additional implementation fee is waived or reduced accordingly.
You pay for the work once.
One Rule Is Non-Negotiable
Responsibility requires authority.
CMI will not accept responsibility for a function unless CMI has the information, authority and control necessary to perform it.
Direct access to relevant contracts, claims data, compensation disclosures, plan records and service providers is required.
You run your business. CMI manages the health-plan functions entrusted to us.

No canned presentation.
Broker Independence
CMI works for the employer. Period.
CMI is retained directly by the employer. We do not work through broker channels. We do not share fees with brokers. We do not accept broker commissions, referral fees or other broker compensation.
If an employer currently uses a broker, CMI may communicate with that broker as necessary during a transition or while reviewing the existing arrangement. That does not create a partnership or compensation relationship between CMI and the broker.
CMI independently evaluates the broker's compensation, contracts and services just as it evaluates every other plan vendor.
You May Already Be Paying for This
The difference in accountability is not.
Many employers already pay substantial annual broker and consulting compensation. Yet the employer may still be responsible for monitoring claims, contracts, PBMs, TPAs, stop-loss, vendor compensation and the financial operation of the plan.
CMI is structured differently. Where appropriate, an employer may replace or substantially reduce existing broker or consulting expense and use those dollars for direct health-plan management.
In some cases, the difference in net cost may be surprisingly small.
Someone is actually managing the plan.
What You Will Never See on a CMI Invoice
- No PEPM charges.
- No percentage of savings.
- No commissions.
- No referral fees.
- No broker revenue.
- No vendor compensation.
- No insurance overrides.
- No hidden revenue.
- No surprise fees.
The price you approve is the price CMI gets paid.
Why Fixed Fees?
Aligned with the employer — not the expenditure.
Your advisor should not make more money because your healthcare costs are higher.
And no one should earn a percentage of “savings” simply because a hospital began with an unreasonable bill.
CMI's compensation is established before the work begins. Our financial interest is not tied to how much your health plan spends.
Not Sure Where to Begin?
Start with the Independent Health Plan Review.
We will tell you what we find. If everything is working properly, you will know that.
If it is not, you will have the numbers, the documentation and a path forward.
What you do next is your decision.
